Insurance & payment

Insurance information

Dr. Harb’s practice accepts most major private insurance plans and Medicare. Coverage details vary by plan, so we always recommend confirming the specifics for your individual policy.

What we accept

Plans we work with

We accept most major private insurance plans and Medicare. Examples of the carriers we most commonly work with — this is not a complete list, and acceptance is not limited to these plans:

  • Blue Cross Blue Shield (BCBS)
  • UnitedHealthcare (UHC)
  • Aetna
  • Cigna
  • Medicare (traditional)
  • Medicare Advantage plans
  • TRICARE
  • Workers’ Compensation
  • Most other major private insurance plans

If you don't see your plan listed above, we very likely still accept it. Coverage details — copays, deductibles, prior authorization requirements, out-of-network status — vary by plan and by policy. The most reliable way to confirm your specific coverage is to call our office, and we'll walk through your benefits with you.

Verifying your plan

How to confirm coverage for your visit or surgery

We help patients work through the insurance side from the beginning. The simplest pathway:

  1. Call our office at (202) 835-2222 or submit a contact request. Have your insurance card handy.
  2. Our team will verify your plan is in-network and walk through your benefits — copays, deductibles, anything relevant to a consultation or surgery.
  3. When surgery is recommended, we handle the prior authorization process with your insurer on your behalf. You don't need to navigate this on your own.
Self-pay & out-of-state patients

Self-pay options

Self-pay is available for patients whose plan is out-of-network, who are uninsured, or who are traveling from outside the region for surgery and prefer a direct-pay arrangement. Our team will provide clear, transparent pricing before any commitment is made.

Self-pay is a common pathway for patients in our travel-for-surgery program, where the entire surgical episode is coordinated through the practice rather than going through an out-of-state insurance network.

Workers’ comp & TRICARE

Workers’ compensation and TRICARE

Workers’ compensation cases are accepted. Our team coordinates with the case manager and employer's carrier on the administrative side so you can focus on the care.

TRICARE beneficiaries are welcome. We accept TRICARE plans for both active-duty service members and retirees, with the same in-office and surgical care as any other patient.
